Documents just released by the Oregon Department of Revenue show 4 employees failed to overlook a fraudulent $2 million state tax refund.

Three of those employees received written reprimands and the other was demoted.

The names of the workers involved have not been released.

This is after Oregon resident Krystle Reyes claimed a fraudulent $2.1 million tax refund earlier this year.

She has since been sentenced to 5 and a half years in prison.
