Medford, Ore. -- Oregon's health care facilities reported more than 650 mistakes last year, 44 % of which ended in either serious harm or even death.
The new report issued by the Oregon Patient Safety Reporting Program says the "adverse events" include medication mix-ups, falls, infections, and erroneous surgeries.
It's the largest number of problems ever recorded in Oregon, but the Patient Safety Commission says that's likely because more facilities are voluntarily taking part in the study.
The program is designed to collect data on the mistakes and share that data so facilities can learn from each other's mistakes.
