More issues for OnTrack

Medford, Ore. — A follow up to a story NBC5 has been following for months.

Oregon Health Authority has identified more issues inside OnTrack

It sent two letters to the drug and alcohol rehab program demanding change.

According to OHA, its most recent check found 11 areas of non-compliance.

Among them:

  • Services and locations not covered by the agency’s certificate of approval.
  • Confusing, disorganized and incomplete human resources records
  • Workers failing to let the Department of Motor Vehicles know when people have completed treatment.

In the letters, OHA said it’s “concerned about the persistent and ongoing nature of these types of rule violations.”

OnTrack has a month to fix the issues.
